Jake Kielb's Hockey Scholarship Program
The Jake Kielb’s Hockey Scholarship Program was created to support local youth hockey players and goalies in need of financial assistance to continue playing the sport they love. Understanding that the costs associated with hockey programs can be a barrier, the goal of this scholarship program is to assist families with the costs associated with youth hockey participation.
Application Period
Applications are now accepted year-round. Funding will be awarded based on financial need and the Foundation’s available funds each month. Due to limited resources, we may not be able to assist every applicant, but we are committed to helping as many families as possible.
Eligibility:

Applicants must be registered with a USA Hockey affiliated youth organization located in Clark County, NV

Applicants must be 17 years of age or younger

Scholarships are open to all youth players, not just travel players
Procedure:
The following items must be submitted via email to ginausufzy@hockeyforkids.org:

Completed application form

Written statement of financial need, including any supporting documentation (e.g., most recent tax returns or other proof of financial need)

Optional: Letter of recommendation from a coach or community member
All application questions must be answered completely. Incomplete applications will not be considered.
Review Process:
Applications will be reviewed within two weeks of submission. Recipients will be contacted via phone and/or email.
Payment:
Winners will work directly with a representative from Jake Kielb’s Hockey Foundation to coordinate payment of scholarship funds.

ginausufzy@hockeyforkids.org no later than the 1st of each month. If received after the 1st the application will be considered for the following month.
